Nice, that must be it, as I completed that sidequest.
( from Bethesda very own support page )
"Killing the Weepers present in the sewers beneath the Hound Pits Pub just before The Golden Cat mission will count against the Clean Hands achievement, but will not count against the achievements dealing with being spotted.
If Granny is Sleep Darted or Choked Out after destroying the Cameo in the Flooded District Mission, this will count as a kill and will prevent you from getting Clean Hands. "
Since sleep-darting Granny still leaves the "no kill" box checked at the end of the mission, one can safely assume there is no way to remember whether those two weepers were killed or not during the inter-mission sequence...
Anyway, I also stole Granny's key and freed Slackjaw in the process, leaving them to their fate, it counted as both No Kill / No Detec for the mission but I didn't unlock Clean hands. Either it's definitely bugged, or I killed one of them Weepers before Mission 2 and didn't even know :(
Spose I'll wait for the patch :V How annoying.
What sucks about this is if I replay and screw up the conditions for it before the patch, then no achievement at patch. So, there goes the replay value.
The thing that makes this achievement so desirable is that it's a memento of what I think is the most unique experience I've ever had in a stealth/action game. Keeping enemies alive is genius, and the achievement is pretty much the only completely unique one I've ever seen. Figures it would be broken.
Then you apparently never played Deus Ex Human Revolution (Pacifist Achievement), DEHR Missing Link or it's 'predecessors', which could all be played through without killing anyone - bossfights excepted. Not to mention the magnificent Thief Series, to which Arkane has a very close connection btw.
No-kill is the only way to really play those games. They're much more challenging if you're not allowed to kill or even to knock out someone.
Just finished my first no-kill playthrough of Dishonored either, but as I gave Granny a good night sleep per dart, didn't get my "clean hands" achievement either. I really don't like the idea of leaving Slackjaw alone with her, so I it seems I won't get it for some time. :/
