They don't count for clean hands, but they trigger the blood effect on the blade.
I *do* get a bloody blade when killing guards or weepers.
I woke up for the Admirals weeper mission in the sewer underneath the Hounds Pit and hit L-mouse and my sword was red. It was a no kill game so no weepers,no civs, no watch were harmed. For the rest of that game my sword was both clean and bloody at times. I remember distinctly that for the beginning of the Boyle Party during the same game that it was bloody too. I think there must be some glitch.
I also don't get the world card for Granny's rune in the Knife of Dunwall Eminent Domain mission every mission. it's very hit and miss. I'm replaying now hoping I get it after two misses.
Problem solved. I think it was a video hiccup between driver releases. it's gone with the latest NV release. Granny Rags world card recipe is back on the table. I guess for some reason the paper on the table cloth wan't being rendered? IDK. It's back anyhow.
Nope, I don't kill rats, I find that the blade is only bloody after I buy the upgrade for Corvo's sword. so I'm thinking it's a glitch/bug with the Blade Crossing upgrade :-/
