I was curious about a couple things you mentioned, that I'd heard before but never experienced so, I just went back to try them out. I saved Emily and did nothing about the Pendleton bros, and waited for confirmation of her rescue before going thru the door myself. The only consequence of that was, when returning to Samuel, he said he'd wait for me to finish with the Pendletons. Then, I did it again, this time I saved Emily and unlocked the door but went thru it before the confirmation message. After going thru the door, I then got the message that Emily was rescued and, again, Samuel told me he would wait. So, as you said, something critical must have happened to end the game.

What exactly did the message say, when it told you that you couldn't progress?
