Ghost, Shadow and Clean Hands - keeping track?
Is there a way to see if in your current progress of the game you've voided any of the requirements for those achievements?

As in, if so far you haven't been detected, killed or killed anyone? It would be easy to miss Clean Hands for example if you've haven't properly hidden a body you made unconscious and that could have later on been eaten by rats counting against the achievement.

There is no way to check whether or not you killed somebody, or were detected during a mission. Your only indication is the stat screen after each mission.

Yes to both, but remember that these screens only give the information about their respective mission, if you were detected in a prior mission, then getting ghost will be impossible, no matter how many missions you finish with a ghost rating afterwards.

Of course there is no mission prior to this one, just making an example ;)

Just to make this clear - an unconscious enemy will count as killed if it gets eaten by rats? Does that count as "detected" too?

If I'm wrong, someone correct me but from reading around:

A body eaten by rats will count as a kill, so you'd be advised to 'hide' them well, as well as not choke them and throw them off a bridge or something.

Detection though only happens when the three indicators around an enemy's head light up, which would require your presence nearby.

Completely accurate (there are in-game counters for each stage of the indicators, but for the purposes of ghost, only filling it up completely and turning it red counts). Dogs don't count as kills though, iirc, but they do for bodies found. Rats can eat an unconscious body at any time; they don't have to be around when you drop the body - you can even be in the next area when they spawn and consume it (same for guards finding bodies, you don't have to be in the area for this to happen).

It is possible to accidentally kill an unconscious enemy with clipping errors (i.e. throwing him into a small container). In the last room of the prison escape, to your left on the floor as soon as you walk into the room, is a few crates that you have to crawl to get into to hide. I tried crawling in there with an unconscious guy over my shoulder, and dropped him there. His head got stuck in the top of the crate and I guess was enough to "do damage" to the body, and blood appeared all over the place as if I'd dropped him from a 3rd story building. So if you ever see blood splatter around the "unconscious" body, you've accidentally killed him, and need to reload.

As for checking previous missions - if you go into the mission select screen (from the main menu), you can see the stats for your first completion of each level for that playthrough by selecting the mission and pressing SHIFT. Edit: It is this first complete playthrough of each level that counts towards the overall game achievement, so if any of the missions shown say that you killed someone, then you have to go to NEW GAME to have another crack at the no-kills achievement. You can't just go redo that level from the mission select screen, you actually have to start a new game.
