I would suggest that you go for MF&S once you know the maps and be prepared to go high chaos on that run.
I would go through the list and descriptions and put anything that requires killing/combat on the high chaos run.
There are story achievements you will get on your first run no matter the chaos level. and if you get Ghost (a low chaos achievement) you will by necessity get Shadow as well (as it's just not being spotted without the further limitation that makes ghost low chaos).
One last thing: due to an early choice that that most people are afraid will raise their chaos on a low chaos run so do not do (it involves poisoning a still). this cuts off some loot required for the art dealer cheevo later on as an encounter does not take place, so it's usually best to do that achievement on an HC run.

I did the exact same thing these days. lol
Turns out that they can't breathe underwater while unconscious. Go figure
I'm going for the achievements too. Doing a Low Chaos run first, all stealth, no kills and without power upgrades (because the 'Mostly Flesh and Steel' achievement) and then another run, High Chaos + Side Missions and stuffs.
I highly recommend this mod Mission Stats[www.nexusmods.com]. It alerts you If you've killed somebody or have been spotted for example. Also, it creates a new option on the menu so that you can check anytime your statistics for the current mission.

That's odd. It states that is not required on their github page.

Edit: Well, I found a way to rebind the quick action wheel or whatever its called to a button on my keyboard, the middle mouse button it was bound to wasn't triggering it.
For the other question, I've been using the middle mouse to change between stuffs
