Ghost/Shadow Achievements bug?
I finished the game recently only some time later, I realized that I didn't get the "shadow" and "ghost" achievements, but I still managed to get the "clean hands" achievement. I checked all the statistics for the missions and they are all marked as "Never detected" or "Didn't kill anyone" (Except for the prologue, of course)

Maybe because I knocked out Granny and destroyed her amulet, so technically "killing" her?

Or why did I infect Slackjaw's elixir and indirectly kill thousands with the plague?

On certain missions, I didn't even knock out anyone and avoided killing even rats or river krusts (Yes, I completed the game without killing even one)

Neither of those would have done it, I did both and got the achievements. It sounds to me that perhaps someone saw you the instant you choked them out or sleep darted them, and it counted as a detection, ruining the rest of the save file. Perhaps a dog? They count for detection, I believe.

The whole "never spotted" checkmarks is odd, but it could either be a bug, or possibly you restarted a chapter through chapter select and got a better result than you did in your save file, overwriting it. The chapter select is strange about what it shows when you have multiple saves.

By the prologue, do you mean the assassination day, or the prisons? Cause the prisons count.

Both, I didn't kill any of the assassins on the day the empress is killed and I didn't kill anyone in prison.

I pretended to play again, but killing everyone this time ... I think I'll play again the same way xd

It's quite possible that you were detected by a dog or a weeper and didn't realise.

What you also have to remember is that whilst the sections of the game in between missions don't have and end mission screen with all the stats including whether you killed anyone or got detected, they still count.

It's very common for people to accidentally kill the weepers in the sewer by their bodies slipping into the water, or get detected by one of them after taking out the other.

You say 'I checked all the statistics for the missions and they are all marked as "Never detected" or "Didn't kill anyone"' which I am understanding as you have either one or the other for every single mission. Unfortunately the wording for the achievement is not great but to get Ghost you need to have not killed anyone except key targets (you got clean hands so this isn't an issue) and also you must not get detected on any mission at all, unless it is by the key target (I have not personally tested that though.)

The wiki page can hopefully help clarify this a bit too: https://dishonored.fandom.com/wiki/Ghost

I found out what I did wrong, there is an achievement (Vanished, You escaped prison and navigated the sewers undetected) and only then did I notice what I did wrong, because I didn't have that achievement ... now it looks like I will play again from same way. Thanks for the help, if it weren't for that comment I wouldn't have figured it out anytime soon

So I can play the same way I did last time, but can I just kill priority targets instead of neutralizing them?

From what I understand, yes. However I personally got both achievements at the same time and didn't risk being detected at all so I can't guarantee that's correct.

I went through the whole game undetected and didn't kill anyone, double checked the end of level summary and both boxes were ticked for every level. Nothing flashed up at the end of the game.
