Will doing the second Granny Rags quest ruin my no-kill run?

I'm pretty sure it doesn't - as it is a special action and not an actual NPC kill.
However, it increases the Chaos and will significantly increase the Weeper count for the following mission at the same location.

i still wouldn't do it just to be on the safe side if it does.

Doesn't doing the mission get rid of the non-lethal option for House of Pleasure? Or does slackjaw survive?

Although I haven't actually done that quest i'm pretty sure if you do actually do the second granny rags quest you can't get the non-lethal ending for the following mission, since I remember someone saying in a video guide for the no-kill run the next time you go there, there would be only weepers.

Edit: I'm also pretty sure if you do the first quest you have to kill granny rags or let slackjaw die at one point in the game, I don't know if this counts as killing someone, but just something to keep in mind.

I'm pretty sure I tried to knock her out before, she just teleports and leaves rats behind, appears somewhere else.

well nevertheless, if poisoning all of the bottle street gang doesn't get you any kills, leaving slackjaw for granny rags probably wont either.

It might not, but basically to answer the OPs question, if you're going for a no-kill run you should avoid the poison quest, as it would ruin your ability to get the nonlethal ending for House of Pleasure.

In my first playthrough I did the second quest for the reward. Slackjaw was still available for the non-lethal in House of Pleasure.
It will make the confrintation between the two very difficult to solve non-lethal. Not impossible though. I watched some vids.

You can make Slackjaw kill her an keep your own hands clean. Tricky though.

I have completed the game with 0 kills, helping Granny contaminate the still and also having Slackjaw get rid of the twins. Take this with a grain of salt, as I have heard these achievements are a bit touchy. What works for one person won't necessarily work 100% of the time.

Contaminating the elixir still does not add to your kill count. I think it's because there aren't actually any deaths because of it until the mission after, but they still don't count in that one.

Once you contaminate the elixir still, there will be a bunch of weeper-ized gang members when you go to see Slackjaw during House of Pleasure. If they spot you, it will ruin a ghost run. If they get killed by or kill the other Bottle Street boys, it will not count against you. Slackjaw will be alive, and will tell you about how someone is plotting against him and poisoned his still.

Once you get to the Flooded District mission, if you have completed BOTH of the aforementioned side quests, you'll have a sticky situation to deal with. There is a way to do this with zero kills and not being spotted. Granny Rags has captured Slackjaw and asks you to help cook him in a stew. He, of course, begs you to save him. If you knock either of them unconscious it will count as a kill. (Knocking Granny unconscious counting as a kill is a bug). Granny is carrying the key you need to escape. If you pick her pocket, she will turn hostile. I got around this by slowing time, grabbing the key, and teleporting out as fast as I could.

First of all, to trigger the Granny/Slackjaw confrontation in mission 7, all you need to do is simply talk to Granny in mission 2. You don't actually have to do either of her jobs.

If you do her jobs, you can take out the three Bottle St thugs, non-lethally, and infect the Distillery still, without any consequences regarding the Ghost and Clean Hands achievements.

There are basically, three ways of dealing with Granny and Slackjaw in mission 7, while preserving the two achievements...
  1. Defeat Granny yourself. Burn the cameo, take out Granny non-lethally, and release Slackjaw. If done correctly, you can escape the area with 0 kills and 0 detections, satisfying both achievements' requirements, plus getting Slackjaw's 500 coin reward as well.
  2. Help Slackjaw defeat Granny. Steal the key and release Slackjaw to fight Granny. While this method is rather entertaining, it will only work about half the time. Granny is a formidable opponent. If it does work, you'll also get Slackjaws 500 coin reward.
  3. Steal the key from Granny and run (or blink, to avoid being heard by Granny). This is the easiest and safest way to deal with the situation without detections or kills* although, you won't get the 500 coin reward from Slackjaw.

* It's interesting that, stealing the key and running away will not result in a kill however, either of the two versions of Helping Granny, will end in a kill.
