Ghost and clean hands

Is there any bonus for ghost and clean hands?

Well I think there are for some things, but it's more for yourself I guess (and overall I find it more fun then just killing them) =P

Well, to get Ghost you pretty much need Clean Hands anyway, so... You might as well bag both at once.

Technically, there's sorta a reward since that playstyle leads to Low Chaos, and Low Chaos means less enemies in general and a nicer ending.

There's no in-game rewards aside from that, though.

I don't know if it's a bug, but i've completed the game with Clean Hands/Ghost twice now, and i didn't even get the achievement.
It's a pretty nice challenge but there's nothing really special about it apart from the ending, which someone else mentioned before.

No, no rewards. You just get the reasonably difficult (in my opinion) achievements. I had so much fun playing a Ghost/Clean Hands/Mostly Flesh and Steel playthrough on Very Hard difficulty. If you can do that then it kinda makes you feel good about yourself. Once you deeply familiarize yourself with the way stealth works it gets pretty easy.

Did you alert/kill the weepers between missions 2 & 3? (they don't show on any mission stats screen) . Sorry if you have already thought of that but I nearly always get clocked by one of them on a ghost run :) .

Regarding in-game rewards for clean hands there are some targets that get you rune or coin rewards in-game if you don't kill them.

Actually, there are a couple of rewards. Two runes come to my mind, but there might be more:
One for sparing at least one of the Boyles, the other one for not killing the Twins

the first you mention is a rune but the second is a coin reward, you get the rune for doing the duel with lord shaw
