The fourth rarely shows up unless you get fully detected by one or more of the others, Where I really got shocked by an extra whaler was in the flooded district in the first building where you are being held. I was playing flesh & steel and decided my only way was to go full out, got one, the other saw me and then a third (who was likely watching from the rooftops across the street or something) showed up apparently that was the only time I had made enough noise to draw him in.

I think some of them spawn from nowhere only when you are detected.
Ok, now THAT's a first for me. I've never had that happen, although I've also never went hand-2-hand with them to make noise enough to draw attention. Interesting. I'll have to try that sometime.
Oh... and he's always been there whether I was detected or not.
