Killing ANYTHING but hounds, rats, and river krusts counts as a kill! So, even Weepers count as kills as well as assassination targets, civilians, and, of course, guards/witches/whalers.
The nonlethal option for Campbell is to sleepdart him and then carry him to the Interrogation Chamber and strap him into the chair. You can then find the Heretic's Brand on a desk in the small alcove overlooking the Interrogation Chamber and brand him. That will cause Campbell to be kicked out of the Abbey and conclude his nonlethal elimination.
Just sleepdarting him and leaving him is not elimination. Think realistically. He would just wake and go back his evil-doing. You're out to either end his life or end his career.
I watched a master thief "prenatual youtube" do a Stealth speedrun, clean hands, ghost, no powers, no blink, no choking anyone.
Understanding that you're going for Ghost and Clean Hands achievements, there are two ways of dealing with Campbell. As mentioned, the branding solution is the most common however, you can get Curnow to kill Campbell for you, which will NOT count as a kill against you. But, just remember to grab Campbell's journal before leaving Campbell's Secret Chamber or Samuel won't be at the dock.
Where could your 1 kill be? If you verified each of the unconscious bodies and didn't leave them until they were stable (not moving or shaking at all), then it's not likely someone slid down stairs, slipped off a roof or into some water. And, no need to worry about naturally occurring rat swarms. If they eat an unconscious body, it will NOT count as a kill against you.
The first possibility that comes to mind is if you rewired a wall of light on Clavering Blvd. If a guard walks through it and dies while you're somewhere else, it will count as a kill and you wouldn't know it.
If you render someone unconscious near a damage source, they could be killed. Be careful around any open fires.
If you close a door on the head of an unconscious body, that can kill him, although it's rare.
When going for those achievements, I usually recommend avoiding contact with anyone and finding the high ground whenever possible. Stick to the roofs and ledges, using Blink to your advantage.
I have done as Eternie suggested and watched videos, and beleive I have seen the one referred to. I prefer not to watch too much, spoliers and all, and I like to try to find my own new ways, to me. Believe me, I only give up and ask for help if I have no hair left to pull out :)

I'm not sure about the 3 overseers, that could be possible. I didn't go into that building in my ghost run.
But I'd like to offer this great mod that allows you to check Stats mid-mission, like in Dishonored 2. It's very helpful for ghost/clean hands.

Same as if you watching someone get eaten by a swarm of rats isn't attributed to you unless you caused the swarm by summon the rats, that then would count as a kill to you.
It's all common sense really.
