Radiation is bonkers.
I will be going to my mission and out of nowhere the radiation sign begins to flash and I am dying. This has been ruining the entire experience. I just started playing, and I'm still in swamp and I know the water is radiated but even when not in the water just out of nowhere on my way to a mission I will begin dying. Its so random, and frustrating and I had to load back all the way to the near start.

yeah, the game is pretty brutal and (enjoyably) tough to start with.
as far as radiation is concerned.. not gonna give anything away but you will find better things and ways to deal with it as you progress.

my advice is that you try to remember where the radiation hotspots are and either avoid them or run like buggery through them. if, or rather *when*, you are irradiated, take the anti rad "medicine" to get it to zero, or drink vodka (you may have to neck several bottles so try to make sure you are in a relatively safe area - fighting while hammered doesnt work out well!

oh... and save often!

The main problem I had with clear sky was that the radiation (and I think the stamina) bar was stuck in the inventory screen, and that I had to keep going into my inventory to check my radiation level was just annoying.

you have the vu-meter indicator on the bottom right of the screen. The needle shows the radiation level in the area and there is a red line (gauge) that shows how iradiated you are.

I dont expect it in a beginning area with no warning, and being unable to prevent it, no.

You do have warning and you can prevent it. The warning is underwhelming though (the needle of the vu-meter going up) and the "preventing" consists of avoiding the radiation areas by keeping a close eye on the stupid vu-meter, and/or find and equip the right artifacts....and vodka :P
For all its worth, in real life it was much worse....

Yes but when you accidently stumble into it, it's like a death sentence if you don't carry an entire bag of vodka. Its very difficult (which isnt bad).

there are somethings you can do at the begining of the game to avoid high doses of radiaton; pay atention to geiger counter sound, if it starts going bonkers, turn around and find another route. Avoid getting close to any kind of machinery,like broken down vehicles, they tend to be radioactive. Avoid going inside buildings for no reason, interiors can also hide radiactive hot spots. Avoid difficult anomalies, such as the burner anomaly, until you have a better suit. Hold on to artifacts that remove radiation as a priority, such as bubble and jellyfish.
Radiation is one of the key dangers in this game, along with getting ambushed by who-knows-what-it-is... it's challenging, you have to take it slow when starting out, it's tough, but once you get past the first map, it gets pretty good. Definitely worth the time to play, carefully.

Listen to the geiger counter more closely. Once it goes crazy, back up immediately from where you are. Simple as that. Then go around.

This is probably my main complaint about this game, there is nothing to listen to. The geiger counter does hardly any noise, you have to actually watch the vu-meter.
In SoC and CoP you can get away easy by listening to the crackling sounds, in CS when you hear the sound its too late, you are already in the red.
