I think Ive have only seen it happen a few times (but I have played this a lot 926hours) again I am sure its not just a fed thing, but a bug in the ai in general, as I have seen that with klingons I believe.
Though it sounds like your hitting it a bit more often that you should be, while a pain I would give the whole game a complete re-install just to be safe, I certainly cannot narrow it down to one or two things to try, as it does appear to be a bug in the ai.

Same thing happened to me (v1.94) on the 'Q's Judgment map' today. The federale colony frigate stranded in asteroid belt (maybe, AI assumed the colony frigate to be capable of capturing neutral extractors, instead of the carrier frigate). At first it had started normally by colonizing one asteroid next to their homeworld, but then it just bypassed another one. AI never went for other planets even with their Ambassador colonizer cap, and never researched tier I planets, although it had eliminated the militia.
Instead it researched the envoy cruiser and (self-)relationship techs. Weird.
i should add sometimes when it looks like they are not doing anything, if you scroll about you may find they actually have a fleet chasing some of the enemies, however they are miles and miles away from the planets and just flying off into the middle of nowhere, but because their ships can only match the enemies speeds, they just keep flying, however as they have not killed everything in that gravity well, their coloniser wont colonise as technically there are still enemies about.
I have seen that happen a lot with ai. Where the ai has sometimes not just sent a few ships, but actually sent their entire fleets after some militia that are just flying off into the distance, with newly built ships just joining in the huge convoy of ships, leaving all their planets unprotected and leaving a lot of uncolonised planets behind.
They had cleaned the militias, but moved on, without colonising the asteroids/moons.
Although, in STA3 it's safe to colonise before destroying the militia, because the latter doesn't boast any siege vessels and is going to ignore the construction frigates as long as your fleet remains in the well.
I always start by colonising before fighting the militia to get the income up asap (or if - as Romulans - I want the construction frigates to build mines in order to help defeating strong militias. but that is taking some time).
I do that, but I do not think the AI does, as I say you may not notice and think all the militias have gone, but sometimes if you scroll the playfield you can find vessels miles away, chasing down the odd retreating ship, but not actually being able to destroy it as its just out of range, so the ai either continues to hunt them militia off screen, or moves onto the next area without colonising etc.
