Depending on the mod (soage is a BIG offender) sometimes if you send your fleet they will go to the next planet with their normal drive instead of jumping. If they do so, they can still fight enemy ships in the next planet, but count as being in the previous planets gravity well (it also takes freaking forever to get to the next one).
One of the best ways to make sure this doesnt happen is to click on the planet itself when sending move order (if you try to send them to a specific place before the jump, there is a much higher chance they will bug).

Well at least you know the "fix" for the issue, good luck with that.

Another possibility is the Titan is not pinned to the Empire management tree. Ships that are spanwed in like SoGE are not automatically pinned. You can pin any ship to always display in the Empire Tree by clicking the push pin icon when the ship is selected.

What I got from what he said was, he sent his entire fleet including his titan to attack a planet. The titan and SOME ships jumped, the rest didnt jump so his titan ended up dying.

If that is the case the ships should be visible in the planets UI. I'm suspecting he more lost track of where his titan was. Either is a possibility so more information is better.
