As for why you can't create the kingdoms, I'm not sure. They should have become titular kingdoms and still be formable, maybe you don't meet the culture requirements for them? (I think the kingdoms of Greece and Bulgaria require you to be Greek and Bulgarian respectively so that might be why you can't create them.)
do you know if the cultural requirments for a kingdom are still there if i form an empire?
i dont want to form an empire and have kingdom title with 80 percent of my territory associated with it. i imagine if i hold that title and give out kingdoms all the dukes in mega-hungary will get pissed
