Creating titles is definitely required to stay under vassal limit. I often hold back on creating an Empire until my crawn laws and succesion are set the way I want them too. Absolute crown authority, max feudal levy, et cetera.
Even if you are using gavelkind succession and the game is going to create those extra kingdom titles for spare heirs anyway, it's still better to let the game do it than to have those angry dukes. They won't be angry at their new king unless he ends up with multiple king titles. Just make sure you have an empire title for your main heir before you die.
You are right but you have to balance that with the fact that if you do not create the 2nd kingdom titles within 100 years all these duchies will be the de jure part of your kingdom and therfore have a say on the succession. try to create the 2nd kingdom title every 75 years or so to avoid this issue
I've had the scotland kingdom title become landless and no better than than giving someone a cupbearer title
