Only on trying to exit the game via normal means, Jan 4th and July 4th. and unless you specifically tell the game to save to cloud, it will save ironman games locally.
After double checking I can confirm Ironman does save after decisions and after several events such as birth of your child or death of current character. Saving after battles was indeed a mistake.
Have an Ironman currently and my King died.
There was no autosave.
