How does someone get the infirm trait?
I'm asking simply because i want to avoid it. Byzantine Emperor, expanded very far, then my 48 year old emperor gets infirm and then incapable. He soon dies and the throne is given to this 6 year old son. Crown authority is reduced to nothing and then a civil war erupts and destroys the empire. In other words, a random trait given to a generally healthy character destroyed hours worth of gameplay.

Any way to avoid infirm? And what causes it?

My first guess would be "Age". And thus not avoidable.

All the big empires have the issue that they are barely holding together and will collapse the second there is a ruler change.

The best way to stabilize an empire is to make certain that all counties are in the same Culture and Relgion.
Also keep/take a big demnesse as possibl.
I sometimes killed my subjects so I would inherit thier counties/duchies and get closer to demnesse limit.

Infirm can only be gotten by an event with the requirement of the character being at least 50 years old. You can't really proactivally avoid it, it just triggers sometimes.

Got some bad news. Your character doesn't have to be at least 50 to be infirm. My 17 year old heir just got the trait out of nowhere :(

I think you can also get infirm from recovering from incapable

Well you can still get it from a wound not healing. But the event to get infirm directly can only be gotten +50.

Poor health characters can get it before 50. I once went on Hajj, shared water on a ship (-1 health) and then shared food (either -0.5 or -1, don't remember). Soon after the Hajj my character got infirm, he was like 30 and didn't have a wound.

I've had several characters get the infirm trait before the age of 50 with no wounds or other health traits involved.

The game sure does love to throw surprises in. My Emperor of the Roman Empire got incapable when in a battle with some paulicians (Paulicians were outnumbered 3 to 1) and his 12 year old son came into power, then bam, crown authority put to the lowest, rebellions everywhere and game over. Hours of gameplay gone in 10 minutes ♥♥♥♥ my life :(

Yeah getting incapable from battle is always a possibility. I had an awesome emperor (created title of spain, france, england and germany) get a whack on head at 26 fighting the Emperor of Carpathia. So annoying as he had a marshall skill of 38. Died 2 weeks later leaving 2 year old son in charge and yeah Bam lowest crown authority and rebbellion to put elective monarchy in. Thankfully I won and when I got to 16 I had my regent imprisoned. He died 40 years later still in prison. Ha

That's one way to punish the regent who brought the empire to rubble.
