It's not too difficult to mod clothing, and since it only changes the interface folder I believe it should be ironman compatible. Here's a basic walkthrough.
1) Make a mod folder (check the CK2 wiki if you don't know how to do this) and .mod file. Your mod should have an "interface" folder in it.
2) Go to CK2/interface/portraits.gfx and make a copy of the file in the [yourmod'sname]/interface folder.
3) Open it in a text editor, and find the "# Muslim female" headings (there should be 3, one for each age bracket).
4) Each line underneath is a layer which is used to construct the finished portrait from sets of assets. Some refer to clothing, some refer to facial features.. it's pretty obvious which is which. For each clothing layer, simply add "_early" to the end.
i.e.
"GFX_muslim_female_clothes_behind:p3:c0"
becomes
"GFX_muslim_female_clothes_behind_early:p3:c0"
Save the file and load the game with your mod, and Arab women should continue to use early clothing all the way through the game.
Alternately, just marry women of different graphical cultures.
