If you have that, it's probably, as Yaldabaoth said, that you don't own everything, so there will be at least one barony held by someone else.
Yes a screenshot (post it here) would be best.
As Lankard said/implied, threads like this pop up frequently and it usually is either a misunderstanding about the requirements (CK2 achievements can have less than stellar descriptions) or the person is missing a barony here or there (checking every province for every barony is tedious and it's extremely easy to miss one).

It lists all of the de jure duchies you need to control. You need every de jure county in the duchy and any towns, tribal areas, temples must be owned by you or a vassal or one of your sub-clans.

Other than that, there must be a temple, castle or tribal holding or town somewhere that remains independent. It won't show on the map. You'll have to look province by province and find it.
Check Jemotwona first, though.
