Edit: Plus if the guy had a kid with his horse wife then at the very least some horses aren't infertile.
AFAIK horse trait:
a) give you -1000% fertility
b) prohibit you from holding ANY title.
So either you're playing modded game, somehow abused console (and it could broke everything) or "stupid" events have own horses using own traits&rules. In that case - sorry I never play with stupid events on after looked into files what they're doing :)
Then post some screenshots lol. If you're 100% sure your "heir" lacked horse trait then we need to check your succession laws & dynasty. As if pony WAS your heir but you get game over - this mean he wasn't member of your dynasty. Martilineal marriage? Bastard?
So there are horses that do not have the horse trait? What makes a character a horse?

You get fertile horses by them joining your court randomly, they will slowly add more horses to their court, marry a horse, and have horse babies
