Problem is that the forged bloodline is only available if you don't have any bloodlines at all. Very stupid and punishing design. Though beyond that, most bloodlines from events or feats don't have this restriction.
Warrior lodge is only available every 100 years, though i think if you die during the legendary raid, you still get the bloodline. So technically this would still have been achieved by the same ruler, even if dead at that moment.
Catholics can also get a saint bloodline from direct ancestors, if they don't have one already. I think you need around 2000+ piety and good traits, then it's almost safe to become saint posthumously. Theology focus helps with both.
You would need an heir with enough prestige to forge a bloodline, before the ancestor is sanctified. Usually he gets "Blessed" status first, then some years or decades later Saint.
Muslims, Buddhists etc. are probably stuck with that one forged bloodline.

Here it is, was a good run too. I am you could theoretically have an unlimited number of bloodlines.
To be clear, the question wasn't "how many bloodlines can you have", it's "how many can a single ruler establish" two totally different ideas there folks.
That's exactly how we answered you. Some players have limited experience and didn't provide an accurate response. You can establish upwards of a dozen bloodlines with a single character if you line it up properly.
You can also get a few more with one person.
4) Immortal Blood (get the achievement for immortality)
5) Human Sacrifice Bloodline (Aztec or any reformed pagan religion with human Bloodthirsty Gods)
6) Crusader Commander Bloodline (convert to Catholicism, participate in a Crusade, get 15 crusader commander events)
7) Saint Bloodline (die, be Catholic or Orthodox, have no sin traits, have 4 or more virtue traits, have a lot of piety, don't be a demon worshipper, an apostate, or excommunicated).
Technically speaking, the only limit is the number of bloodlines that are still available for you to forge, since some are prevented if you own others. And time, I guess (unless immortal).
There's also a pagan equivalent of sainthood (ancestor veneration feature), which skips the beatification process and outright gives the blessed ancestor status and bloodline.
There're a few more bloodlines from special actions:
8) Teuta or Phalaris (obviously can't get both since they're gender-specific)
9) Duelist (different from the ancestor worship one)

11) Roman Empire (unaivalable to members of historical Roman bloodlines, i.e. Basil and Alexios)
12) Israel
13) Samrat Chakravartin
14) Saoshyant
15) Gengis Khan (unless the historical one already exists)
16) Alexander/Ashoka bloodline
17) Child of Destiny (unaivalable if you own another ambition bloodline, i.e. Alexander/Ashoka, but doesn't prevent them if you get it first)
18) Lord of Outremer (doesn't conflict with other Crusader bloodlines)
19) Destroyer of Byzantium
20) Children's Crusade (yeah, pretty much impossible, I'm just listing it for the sake of completeness)
21) Reconquista (standard, or El Cid)
There's also a Boudica bloodline that, in theory, should be available to female Celtic rulers of Britannia, but I think it's not been enabled anywhere.
And that should be everything. Of course earning as many as possible would be pretty hardcore, though 6 or 7 should be easily doable in the right conditions.
Then the child of destiny also needs to become immortal. So that's a pretty solid start.
