You have to have control of Poland, Bohemia, and all of the Empire of Russia, then you can form the Slavic Union. Sort of like a Polish roman empire sort of thing. But you need total control of all the provinces in the de jure area or you cant form it. Having a holy order castle, in a de jure land that you can't control without kicking them out, breaks the Union.
Interesting, I might try to do it sometime. Now, do you form it through decision in the intrigue menu? I'm trying to find it in the game files to see what could be your problem.
To be able to see the decision you need your culture to be russian, polish, bohemian, volhynian, ilmenian, severian or pommeranian and be King or Emperor.
And to be able to form it you need 5000 prestige, be at peace, not be incapable and completely control k_bohemia, k_poland, k_galicia-volhynia, k_ruthenia, k_chernigov, k_vladimir and k_rus.
nothing about a Holy Order...
Also it says that if you control k_pomerania it will also be added to de jure e_lechczechrus.
easiest solution is usually to vassalize the holy order so they are no longer independent but count as a part of your realm. some holy orders have scripted prerequisites for being vassalized.
you can usually also vassalize them by granting them kingdom tier title (when you're an emperor of course) - that automatically makes them your vassal. you can later also revoke that kingdom title if you need it for something else.
I suggest this is a bug.

Could be a single holding as mentioned, but also only a higher title, like a duchy title held by someone outside your realm.
