Now that I think about it, I've only ever gotten what appear to be the second and third tier of kowtowing, but that's by taking the 'the Emperor may think well of this' options at every available opportunity. It might be that you don't even get to kowtow at all if you don't take those.

Becasue I'm seeing my realm proser, and not doing anything, if there's no permanent modifier, like levy or cash after I Kowtow, then I may as well do it.
Apart from that, no, no permanent modifiers.
