Does Kowtowing have a negative effect?
Other than paying prestige? Do I have to pay money to him or become some sort of gimp to the Emperor?

Prestige, long time away from court in a regency...
Now that I think about it, I've only ever gotten what appear to be the second and third tier of kowtowing, but that's by taking the 'the Emperor may think well of this' options at every available opportunity. It might be that you don't even get to kowtow at all if you don't take those.

So no permanent modifiers? That's what I should have asked in the first place.

Becasue I'm seeing my realm proser, and not doing anything, if there's no permanent modifier, like levy or cash after I Kowtow, then I may as well do it.

You get a tiered modifier to your character; the first level is +1 grace/-0.1 prestige per month; second is +2 grace/-0.1 prestige; third is +4 grace/-0.1 prestige.
Apart from that, no, no permanent modifiers.
