Apart from the obvious "immortal route", you could make people like you better (avoiding possible assassinations)... try and live healthier (i.e., go for events/get buffs that give you positive health modifiers etc... marshal or hunting focus if you have the dlc).
Make sure you shut the gates when there are disease going around. Kick any and all gluttons out as soon as possible if you have to (to avoid them literally eating you out of house and home) if possible, get the gardener trait to keep yourself (and your ppl) fed longer before starvation kicks in.
Avoid leading armies.
Avoid leading armies and then board ships before a long journey (sets you up for scurvey)... instead, just send your army leaderless, and re-appoint yourself leader AFTER they land.
Still, very little info, a LOT of guesswork here...
Get better traits as well.
Get rid of stressed and depressed when possible using carousing focus.
- Get a good doctor with learning 20+ opt for the well tested treatment in most illnesses (Only necessary and possible if you have Reaper Due DLC)
- Get artefacts that give a health bonus
- Get a good spymaster who doesn't have claims on your titles, is in line for your succession (unless direct heir), likes you and doesn't have traits like "greedy"
- Don't command battles personally (quite a good chance to die there)
White: natural causes
Green: Illness
Red: violent death (murdered, killed in battle / duell, executed)
Oh and if you don't have reapers due you can't get a doctor but you won't need him that much either.
Hunting focus when you get older, getting pets also helps.
