I've only seen it though I've never used it.
I currently do not have that option under decisions, but I have seen it before. I'm just worried my nation is about to implode if I play/continue even for a little bit longer.
For the counties you'll need to manually hand them out. Give out duchies only to counts in that particular duchy, and if there are more than one count holding land in a de jure duchy - transfer vassalage.
Edit: You can do all this in paused mode

I usually go through the creating new vassal thing just because I like to keep my vassals from having too many personal holdings :)
