However< if your not playing Ironman then once you get the immortal trait simply console your age to whatever so your portrait appearance is what you want.
PORTRAIT_ADULT_MALE_AGE_THRESHOLD = 14, -- Male child to adult age portrait switch
PORTRAIT_ADULT_FEMALE_AGE_THRESHOLD = 14, -- Female child to adult age portrait switch
PORTRAIT_MID_AGE_THRESHOLD = 40, -- Middle age switch for character portraits
PORTRAIT_OLD_AGE_THRESHOLD = 60, -- Old age switch for character portraits
Those are the ones you want to edit(mine is already edited so characters gain their adult portrait at 14, mid age at 40(instead of default 30) and old age at 60(instead of default 50))
Of course doing so WILL prevent achievements as your checksum WILL change. (but can still play Ironman to resist urge to console/savescum, just no steam achievements)

I did this and it changed the portrait of all characters in the game (did it to the extreme as my character is 700 years old), is there a way of editing the save game for just my character portrait?
Though seeing this game is done, sounds like a neat idea for CKIII
