That is an explanation, yet it seemed to become more common after Reaper's Due.
Can also go Theologian focus, every time you get the "Go into seclusion" event you can be healed. Or you can join the Devil Worshippers and have it removed that way.
Was getting Great Pox random, or caused by his behavior?
(On a somewhat related note, why was I only getting the option to denounce or acknowledge my bastards when they were born, but not to legitimize them? I couldn't legitimize them by right clicking them either, even if they are in my court and I am guardian. I was a Muslim emperor in India. Do they have to be a certain age to be legitimized? Or does it work differently depending on which DLCs you have? I only have four DLCs.)

Beginning wth 15 Learning a better outcome becomes more likely.
Below that less likely.
For amazing outcome, which appears required for great_pox you appear to need
either at least 18 Learning or brave and hoping for 20% chance of a drastic treament.
Apparently mystic treament with amazing outcome works. if only the side effects wouldn't be potentially so severe..
Hence why likley above mentioned use of pilgrimage or theology focus seems preferable.
