You could use a console command give_title to get it, i suppose.

Is there honestly no way to do it? Will my vassal not at least make the title himself
The problem is that this is bugged. The bug appears to be that, whether you or the AI are the ones who destroy a title, once it is destroyed by anyone in a single play-through, it can never be created again. So you can go ahead and create it the first time. You can usurp it. But if you destroy it, or if the AI destroys it, it's gone. It can never come back.
Hopefully pdx will fix this asap as it is a major game-breaking bug... for even if you are careful and never destroy a title, there is no way to stop the AI from doing it, and the AI lives to destroy and re-make titles all the time. So it will destroy them and then not be able to make them and you will end up (so the reportage has it) with zillions of counties all over the place that cannot be formed into duchies, kingdoms, etc.
I'm still doing my current play-through since I have not hit this snag yet, but I am not putting all that much time/effort into it because of this bug. In fact if this were not my spring vacation this week, I wouldn't be playing CK 2 at all until this is fixed.

There is also apparently another bug that is making nearly all titles tend toward elective monarchy, which isn't necessarily game breaking but people find it annoying because it is so a-historical for many parts of the game world. Plus it makes things boring if every title has the same inheritance rules.
Yeah now that I think of it that is a bug with the elective succession
It reached the point where the Muslim Empire of Hispania became elective
Right.
There is nothing inherently wrong with the AI sometimes choosing elective succession, even if it is a-historical. After all, we as players frequently do things on purpose which are a-historical for fun or for strategic reasons.
But having nearly every nation in the world, every duchy, every county, be elective -- is completely unrealistic and makes the game almost silly.
