Aside from that it sounds like a driver issue to me...Here (GTX770) and 331.82 driver no problem.
EDIT : disable tessellation as well, GTX GPUs had problem with it in the past.
No personal OC (it's OC by Zotac - Zotac GTX 670 power Amp 2go).
Tried to install new beta drivers.. it is worse as it now crashes every 2 minutes (in particular during cut scenes) It also crashed when I tried to change Graphic options in game (reactivation of SSAO).
Tried to disable SSAO, Exclusive Full Screen, Tesselation, Tress FX) it works well (more than 100 FPS) but then crashes without any reason (short freeze, back to windows with sound continuing, then error message)
What are the other specs of your PC?
I think it clearly comes from GPU but below full specs of my PC (if it helps):
I7 3770 K CPU @ 3.5 ghz
16 Go RAM
Geforce GTX 670 - Zotac Power Amp! Edition 2 go
Windows 7 64 bits
Windows on SSD drive
Games on regular HDD
The latest game where I experienced serious issues was Hitman (but was then fixed (but I remember I had to disable some options like SSAO)
I also have crashes in Rome 2 total war (but not very often) - which is inherent to TW series
No other crashe in any other game so far (well a couple in Far Cry 3, but very limited).
Do not know what to do.. having a game which crashes every 3 minutes for no reason is not really fun (luckily there are lot of autosaves spots) and I am getting tired to re launch this game endlessly.
Do not know what to do (tried everything (new drivers, old drivers, lower graphic options, run the game as administrator only). Nothing worked so far..

I5-3570K, Asus GTX670 DC2, 16GB Ram, Win7 64bit, 240GB SSD drive.
Try installing the game on the same drive (SSD) as Windows.
Do not know how to underclock my graphic card (and not sure I want to do this just for this **** poorly optimised game). This is just a shame that it does not work or has not been properly improved since March 2013.
Will wait for new NVIDIA drivers and hope that it solves the problem. Meanwhile I will play to something else and will from now carefully check whether there are any serious issues reported for Square Enix Games before purchasing it.
Nvidia tools from nvidia website enables overclocking and you can down clock it from there. Well that is if you want to try and get the game to work. Problem is even though you have a factory overclocked card the drivers and software wont bother it and see it as default because of firmware loaded from factory in the card. The software from Nvidia will make a diffrence after install enable overclock and slide the gpu bar down 50 - 100 points. You may wanna move the memory bar down at least 50 points as well. That is if you wanna figure out the problem or just replace the card completly if you have a spare or the money to spend. Obviously others with the same card are doing fine its just not that card.
I tried two other games (Rome 2 and Bioshock) and luckily no crash on these.
Will then try the decrease of overclocking as suggested above. I am not really a specialist so could you please confirm what I should decrease (GPU memory clock (currently 3304) or graphic clock (currently set at 705 mhz) or both?

**** UPDATE
Tried reducing GPU Memory clock ; Both tomb raider and Wargame European Escalation crashed. Other games still working. I am quite disappointed (to say the least) that this game crashed both itself and another great game on which I have been playing without any issue untill yesterday when I encountered several crashes in TR.
