It is actually possible to scroll through all the available areas in the Fast-Travel window with everything showing 100% and STILL miss some challenge items.
It seems that some players forget to complete the Previous Inhabitants challenge in Cliffside Bunker? Why? Because that spesific area does not have a single base camp, that is, no fast-travel camp. It only has two day camps.
When you sit down at any base camp and you scroll through the fast-travel base camps on offer, the Cliffside Bunker area is not available to Lara because of this very reason - only base camps will show up on that menu.
What you need to do, is to go to the nearest area that DOES have a base camp/fast-travel camp and then move to the Cliffside Bunker from there. The fast-travel camp you need to go to is Survivors' Camp in the area Shipwreck beach. From there you keep on moving east until you get to the Cliffside Bunker again.
After finishing on 99% twice, I finally figured it out. Mind you, in the Previous Inhabitants challenge (Cliffside Bunker) there is one specifically difficult flag to spot. I had to resort to a walkthrough to finish this one.
Maybe this doesn't apply to you, but I thought I should give my input just in case.
