Aye, that execution icon I saw plenty of times and made use of it, but I still have no idea how many times I incapacitated enemies that way. XD Thanks for the input!
Start a manual count, reaching 25 and not climb the achievement .. there you are a serious problem ...
Aye, some kind of progress tracker would've been really helpful. The Stormguard are the best of all enemies in my opinion for this achievement, but sadly, one doesn't get to face them outside of the few appearances they make late in the game.
I guess I'll start my third playthrough. :-P

This achievement is a pain in the rump. I completed my first playthrough yesterday and that was the only single- player chievo I missed, mainly because I levelled up too fast and unlocked Dodge Kill ability before racking up those 25 incapacitations. It's still possible to incapacitate enemies with Dodge Kill unlocked, but it's way harder. You have to precisely time your hitting the Melee button when the QTE engages. Do it too early and you'll miss completely and you'll have to wait for the enemy to take another swing at you. Too late and you'll kill the guy outright with an arrow to the head, which doesn't count towards the Former Adventurer achievement.
By the timeI figured this out, it was probably already too late to unlock it, given that enemies started wearing armour, which prevents you from incapacitating them in the first place.
So I went back to an earlier save, just after unlocking the Dodge Counter ability and played the game from then on, focusing on incapacitating as many enemies as I could, while not unlocking Dodge Kill to make it easier.
Anyhow, I'm planning on posting my pre-unlock save soon in the guides, to help out those unlucky SoBs that missed that elusive chievo the first time around, as I did.
Oh man, I also acquired the Dodge Counter Kill fast and thought it'd actually help with the achievement. :-S Aye, speaking of swing, a major problem for me at least is common enemies. Compared to those using firearms, enemies that outright go for melee are very rare. And even when I tried the game on Easy difficulty setting, enemies with firearms would still shoot at me at point blank range, making it hard to stay alive even when dodging. And whenever they did eventually try to hit with the butt of their firearms, it was so quick and unexpected that I never managed to even get the counter option to prompt.
Wait, doesn't armour actually help? When I think of the Solari wielding dual swords, spears, and the melee Stormguard, I was easily able to get the incapacitation option to appear after two strikes to their armour.

When I reverted to an earlier save, I started at the beginning of my second visit to Mountain Village and proceeded all the way to the ambush at the gate in Shanty Town before I finally unlocked this achievement. There are a lot of melee guys coming at you there, so it's a great opportunity to incapacitate loads of them. On my way there I focused on dodge countering every opponent I could, even those with bows and guns. Often when you get close enough to them (in their faces, no less), they stop shooting and start swinging, giving you a shot at incapacitating them. I was always ready to strafe to the side when they did. It was a risky tactics, but I wanted to get those numbers up without actually completing the game for the second time. And in the end, it did pay off.
Yep, it's still possible to incapacitate armoured enemies, but it takes at least two successful counters, leaving you more vulnerable to other attacks. The game actually wants you to deal with them fast by using Dodge Kill
Again, I actually found armoured enemies to be easy, strangely enough. But I suck at getting the ones with guns and bows, as you did. But I guess that risk is worth taking.
