for counter-block, you can still counter even though you already start stabbing. Just need to press the counter button at correct time.
for the kill streak, what I do is gather as many soldiers as I can and start killing. so there would be enough soldier for the 5 kill streak.
and also, practise. timing your attack, be wary for enemies attack during kill streak for counter and don't rush. you'll get there eventually.

I'm now beginning to think that I should progress further in the game. That way I reckon Ezio gets better weapons (though the Romani Stiletto has damage 5 and speed 3) and he'll generally become more skilled as well. And if I keep all the Den's open I can then still fight the Byzantines.
But for the moment I will keep trying in sequence 4. I usually get 2 sets of 4 guards to converge when attempting the kill streaks. I just find the more there are the more likely they counter me, so any more than 8 just seems too many.
Funny you mention the pistol as I have never specifically selected it as my weapon of choice but noticed in the stats that I had 80+ pistol kills - guess that explains it !!
you don't need to select the throwing knife for the double kill, just need to hold the button. and you can continue the killstreak after the double kill. did that all the time ^_^
or you can press attack button continuously instead of holding it until you got a kill.
and yeah, I think more powerful weapons is better since they nerf the kill streak from Brotherhood.
Good news - I Got One !!
I took on 8 guards from outside the Fatih Mosque in the North West of the map and thought I got one. It included a knife throw and a block, but when I checked the stats... nothing !!
Then, as I was there, I ventured inside the mosque and tried my luck against the men in there, which included 2 Janissaries. For the most part they thankfully stayed back. I managed a couple of early kills then it broke down, but was joined by another set of guards so had enough non-Janissaries to have another go... and got a 5 streak (no knife throws, no counters).
I'm actaully in persuit of Mehment's Dagger. There's some conflicting webpages on which memory I need to do to unlock that, but the concensus seems to be for 'The Trickster, Part 1' which is the Constantine North Den assassin memory. The dagger has a damage and speed rating of 5, so hopefully it will make things easier.

I managed about one a session and I probably spent around 10 hours chasing this !! I think the biggest help was getting Mehment's Dagger with it's damage and speed rating of 5. I also managed a couple with a counter-kill thrown in the middle. I'm still in the middle of memory sequence 4 and actaully got most against the Otterman guards as the Byzantine guards really were chicken and usually ran off before 5 kills.
