How do I talk to the doctor or merchants?

It should really be a case of simply walking up to their location and pressing 'Shift' to interact with them.

However, I'm guessing you are still early in the game so it may be that you have not renovated their location. To renovate, you need to find the flashing sign (usually on the right hand side of their building) and press 'Shift'. Follow the prompts to confirm you want to renovte that building.

For info, when you look at the map you can tell which buildings have been renovated and which are still to be renovated. On the map a white icon means it's renovated, but if it's grey then you still need to renovate the building.
