The reason for why the portals didn't appear is that you were there way too early. Extraction points appear/activate/open at random times starting around 6 minutes remaining. Typically barely any extraction point is available at 6 minutes remaining.
You did not die, you left the dungeon. If you have died, you would not kept your items and your name would appeared in the kill feed specifying what or who exactly killed you.
There is no "congratulations!" screen or anything like that after you escape, you just abruptly disappear and the end-game screen appears. The game has the same end-game screen with Exit and Spectate options whether you die or leave the dungeon, the only difference is that if you have death-cam enabled in the Options, when you die you also have an extra option to view how you died from a 3rd party perspective, for example from the perspective of the player who has killed you.
