Game timer on the right

Are games always as long as that countdown? The dungeon started to shake and rumble when the timer was low, and at zero I entered the afterlife with health and minding my own business.

yes. Its a timer to get out. In the last minute you will take damage. When it reaches zero you will die, regardless of how much health you have

So every dungeon has an exit?

You can find hte exits marked by blue. Whether its a blue starway marker, a blue portal marker. Theres also an elevator in Golin Caves, and a Raft in Ice Cavcerns that come and go every minute (I think). Also, most exits don't start opening until the last 5 mins. If the torches arent lit outside an exit, its either closed, or been used

remember my first games and didnt know that the exit pop up on the last 5, 6 minutes, they are in blue in the minimap and they activate around 6 mins left give or take
