Private servers are typically outdated from what I hear, however the most popular one floating around is Nilly's Realm. As for getting one, various people have made them, but like I said above, a lot of them seem to be outdated. Please keep in mind that Private Servers may be against the Kabam/Deca T&S, however I'm not 100% sure on that.
It depends on the game. For Realm, I'm not sure, but many multiplayer games have allowed anyone to use their server software (Valve has a dedicated server program for Left 4 Dead 2).
