Does it have spiders?
My co-op partner had a literal anxiety attack during the spider-riding section of it takes two. Is there anything similar in this game?

I haven't finished the game yet.

But so far the only spider-like thing I remember seeing looked more like wooden beetles.
There is a part with spider-like crawlers (Bugs with a few legs and spikes on their asses) so no real spiders. Not for a long sequence. But if your friend has arachnophobia, it could be a problem - especialy if he/she play as Zoe.

It's the fantasy level where you can transform into different creatures.

op, so you want "trigger warnings" and the kind of bs that degrades actual gaming experience, in a game that managed through merit (as providing decent gameplay and storytelling, without preaching) to include two female protags (that also seem to be gay)?

think both: the preaching of dei-lgbt-etc bs, and the "trigger warning" are bad. asking for them and supporting them, without understanding why they are bad for everyone, may be worse. if your friend, or anyone else needs to avoid "phobia specific content", then gaming or even most "normal entertainment" (in general) isnt for them: the most reasonable approach is to read reviews, or ask a friend to do one (if you can handle watching fake spiders, then you can risk playing the game once alone and tell your friend your findings, or watch a gameplay-playthrough before buying it)

also, you aleady mentioned the most similar game to this one in your question. but if you want other coop games that are focused in solving puzzles or focused in platforming, then maybe you can find a few searching with those tags-filters.

if you mean similar to this game, relative to something else, then you will need to be more specific. the more specific you are, the easier is to search.

Yes there are spiders. I can recheck the two scenes when I get back home.

Nothing extended like the spider ride.

there are spiders but all they do is crawl on the ceiling, their only purpose being a slight barrier to your progress. Nothing in your face that I recall
There is a mechanic where you become a swarm of cyber spiders as Mio in a later section of the game too.
