Have you tried loading a saved game from before getting the mission?
If you need a saved game at the same point with the building unlocked please let
me know and I'll add it to my list. - I'm in the process now of collecting key saves,
and have just arrived at the southern map.

I am using my own mod I pretty much stitched together out of three other mods: Infamous Fusion, Tom's Realism+ and Far Cry 2 Redux. Played through the game with all three before and never had anything like this happen.
I don't use Multifixer since I find the pop-up every time I launch it annoying and I already edited in most features it does into the game directly.
As for saving, I wanted to do that...problem is I regularly delete my previous quicksaves and always just use one slot for saving, so I have no save that predates me getting that mission.

Nah you don't have to, besides I got through most of the faction missions by now. As far as I know, malaria doesn't affect anything.

I need to play through a few more times anyway. - Like I said I'm collecting saved
games at key points. - The more the merrier.
Key save points are needed too often; They're requested here and I need them for
testing different aspects of the game more efficiently so I'm compiling a complete
list of saves.
Alright, somehow, SOMEHOW, I completed the mission for the UFLL where you kill the arms dealer on his barge, dropped a bomb on him and I after I kill the reinforcements that came with my buddy I get notified "You have unlocked an Underground location", then I drive over there, kill all the guys again, AND I COULD ENTER! Finally got my pills.
No idea what happened, this is like the 3rd mission I completed since I got the bug.
I see you've made your own combo mod. Does that include capping the frame rate?
Nope, though I only unlocked one other Underground location in that area, went to the doctor in Mosate Selao though but he wasn't there, since I already had that location assigned.
And nope, I just turned Vsync on in the options menu. Never had any of the high FPS issues...I mean I really couldn't, I barely get to 60 FPS as it is on my machine.
I would never play FC2 without capping the frames.
I would recommend capping the frames, reloading your save, and then visiting a different underground location.
...and stop overwriting saves! FC2 isn't stable enough to only have one save file.
Well as I said I always have Vsync on, so it's capped at 60.
As for the bug, It fixed itself...somehow.
I actually never had any significant issues or bugs playing the game before, aka beating it like 50+ times over the years. Though me constantly editing values might have something to do with it.
Seems like a moot point in this instance, though. Glad to hear it worked out for you!
Really? I've only ever had the jumping NPC glitch once on my gaming rig (I miss you, gaming PC) and I fixed it immediately by turning Vsync on. Never had any other bugs nor crashes in over 12 years of playing this game.
