as far as I see in chapter 2 you allways have a mirror in cabin, just aproach it

but yes, an updated guide for all the new achievements would be nice.

The Adversary - I didn't drop my blade or anything. Key point was being overt about your intention (killing her). I said I'm here to kill her when I was going down the stairs and once down i didn't stay silent only confirmed my intent. Then got down to the deed ending with double K.O. / kill getting this princess.
The Damsel - you can talk all you like. The key point is to stay commited to saving her - so going down the basement without the blade, later using it to help her and resisting the narrator twice.
The Nightmare - only thing that matters here is keeping her locked in the basement as your final call. Whether that was the plan from the start or a result of failed assassination, that matters not.
Prisoner - same as Damsel route, except for that you go down into basement with blade in hand, which sours the first impression ending up with this version instead of the Damsel.
The Tower - I think it's any version where she just beats your ass without taking much damage herself, be it to your poor preparation or you just giving up mid-fight.
I bet you are right. I just rushed every route and tried to make it as short as possible while making notes.
