Where do you find his plush, i have no idea at all.

Yep, any run, and only then when you give him that plush you unlock the run with that plush.

You get a quest where you have to take it. Just keep talking to him. I think it can randomly spawn before then as an item in store, post battle, etc. but you will get a quest for it.

I completed the plush quest, but it gave me some sort of error message when I turned it in. Was there supposed to be some sort of followup? Do I need to start a new save file?
