Fully upgraded weapons from the beginning of a fresh save??

I started a fresh save file for this game, not NG+, and as soon as I got the Vorpal Blade I somehow had access to all of the fully upgraded weapons. This... shouldn't happen, right? Going into the equip menu on the main menu only shows the weapons I should have unlocked.
I do have a NG+ save with all the upgrades as well, which might be why the game is doing this. But if I wanted to start the game with all of the weapons upgraded and unlocked, I'd play on that file instead.
I did start this playthrough via Texmod with Omri's tweaks mod installed, but that shouldn't have any impact on the gameplay itself, just textures.
Searching for this doesn't bring up any meaningful results, so I'm posting here wondering if anyone else had this happen or knew what was going on. My theory is that the game is confusing this save with the NG+ somehow, but I don't know how or why. It's not a huge issue I guess, but I did start a fresh file in part because I wanted to do the weapons progression and collectibles from the start again. It's just... weird

I've recently started having this issue, too. I have two completed playthroughs, one on Normal Mode and the other on Hard Mode, I went to start a third new game on Nightmare Mode but the only thing that's fresh is my health; all my weapons are fully upgraded and unlocked. I did change the max refresh to match my 4K monitor which 240 hertz but that's all I changed in the game files. Any help will be greatly appreciated.

I found a solution! After much digging online I found one suggestion that fixed this issue. You have to go into the save game folder and delete everything, the folders with any saved games and the Profile folder. This will reset your Profile setup, PhysX and monitor aspect settings, etc. and you can start a new game with health and weapons back to beginner level.
