What was in the vent?

Ok, I am going insane and don't know what mod pack included this weird event. I cannot find it online and I did not clip it. So, I'll do my best to give a play-by-play. Some of the bigger mods I have is Diversity, Ghost Codes and Lethalresonance. I also added in a few monsters, like Football and Rolling Giant.

I mainly noticed what happened when Lethalresonance was added in. Or at least heard it more because its a sound overhaul.

Ok onto what happened:
I was testing out the mods and went to Experimentation. While going through the maze room I heard what could be described as low growling monster in the vents or something heavy slithering around. (Not the thumper). I kept going and found myself in the biggest facility room. I hung out on-top of a shelf and just wanted to see if I could get Ghost Codes lights to work. Which I think made the insanity meter go up. Because Diversity activated and I got the invisible dude that disrupts your vision. After he went away and a few facility light shows thanks to Ghost Codes, I ending up leaving and heading towards the maze again. As I was leaving, I noticed what looked like Jeb's tentacle clipping through the wall. Like just his back half. On the otherside of the wall was a vent. I heard the monster in the walls again and when I ran past the vent, that said tentacle tried to come out and grab me. As if I was at the company building.

Now... I sit here confused because I cannot find ANYTHING in the mod list that would cause tentacles or jeb in the facility and Google/Youtube has been useless.

What happened? Does a mod add a tentacle monster into the walls of the facility? I cannot trigger it again but I can hear the monster as I am playing with friends and so can they.

Clarifying: The sound was coming from within the walls and pipes. It sounds like something large is moving in the walls.

Edit: Found it in the config, the monster is called the Watcher and you can disable it
