In vanilla, it only turns your own body into a masked.
There is a rare chance that masked will spawn on certain maps regardless of what the player is doing though.
Giving them both different effects:
The happy mask just has it run at you and punch you once lets say then runs off laughing.
The sad mask stares at you and will mess with your vision, if you return stare for too long, like the brachen, it'll chase you.
2 shovel hits to kill them, both happy and sad have a chance to both spawn on all maps at the same time, that chance is higher for rend and above, and will spawn more of them etc.
Only having them show up on the rend and higher maps makes the monster diversity sucky for the other maps.
