Basically, if you're a FPS player, you'll check conrners and behind you regulary, and end up with this "note".
It's diferent from "Sanity" which is a hidden stat. When you are alone and when you move your camera a lot (interpreated by game as panicking), your sanity will drop. When low sanity, you'll start to hear creepy musics and sounds, voices... and so on.
If you are experimented enough to be the lone guy inside the facility, you'll hear them a lot.
