Smoking was as normal and as accepted as was drinking coffee, and guess what: lung cancer was almost never heard of.
Second, excluding cigarettes "for the children" from a horror game involving murder is as ridiculous as it sounds.
Third, the historical component is exactly right, and it would technically be exactly on brand for the era. Pretty much everyone smoked everywhere at all times when I was a kid. It was impossible to escape and everyone smelled like cigarette stench all the time, hence my disdain for it now. Few things maker me happier than witnessing the oppression of smokers.

Same amount of fun, no negative health effects
That would be so random. Doesn't even serve the gameplay in any way. This is a horror game not an immersive sim lol.
