A few things left to find (Spoilers)

Hi all!
There are a few areas I haven't been able to find / access and I'm not sure if it's still bugged or if I'm just missing something. Any help will be greatly appreciated! (I will add screenshots soon)

1.)
The Door behind Theo. Not sure how or where to get the key.

2.)
The Door in the bottom right of the Hidden diner. Not sure where to get the key

3.)
The Headless Denzel is just standing still in whatever area I first found him. I get the prompt to talk to him, but nothing happens. Is he supposed to come back to the hidden diner with me? I have a few empty rooms still...

4.)
The giant door to the left when you get to the Golden Tower. I'm pretty sure it's a door but I haven't been able to find a way to access it.

3 is a bug as far as i know. He does moves.

2 and 4 I might think are for the DLC or a squel after the credits.

Well I've beaten the story and haven't seen anything change... Unless I did something wrong.
