edit: I didn't realise this was a new achievement; you definitely need to be early in a new ascension without the upgrades to enemy/firefly density

Even then it doesn't seem to work. MC just hits default samurais offscreen.

It worked for me, ascend, kill the ones u see and instantly press Shuriken, worked 5/5 times, dont even wait 0.1, just kill and press it, works fine then.

There's definitely a little bit of luck involved; keeping your speed as low as possible helps, and you might want to do a medal upgrade reset to take out the speed and firefly spawn from there. Otherwise, yeah, you want to wait for a group you can take out in one click and pretty much hit 'W' to shuriken in the same moment. Crane and Snake will speed things up a bit by reducing CD.

Aha, not sure if there was a ninja patch or if you misread the achievement @Fokeens, but it didn't need to be in a row. Just five misses total.

If you are early in a run/ aren't buying upgrades yet you can time it for right after you've cleared a clump of samuri. Sometimes there is a firefly just off screen but you will get a few blanks. (sometimes the spacing of enemies is too close but it's uncommon.)

On top of what others said:

Try using the Crane pet (at least level 10), that one has a chance to do two shurikens in a row.

Try to change the window size to a narrow portrait mode.

And to be explicit about the "early in an ascension" bit: ascend (not perfect ascension). do not buy any buildings or research, and now enemies come a lot less often and you're a lot slower.

its not impossible, just ascend, dont level any upgrades. make sure to not have any pets/masteries that give speed step boost.
kill an enemy and press W right as you've killed the last mob, it should use shuriken before next wave spawns. might have to play about with the timing but its totally possible.

I just got the achievement, while doing an only buildings run. Speed Step 13 and auto shuriken. Just seems to take a while.

I'ven't tried yet because I'm still perfect ascending and aiming to other achievemnts... but I think is a good idea to have a crane level 10, since it has 15% to reset shuriken timer, so use it and in those case when it resets, use it immediately and it should miss everything... I think
