The people who think it's just a sack of organs seem to forget the lead up to the escape mission is basically 621 limping their way to the Jailbreak unit Walter left iirc.
I'm actually more curious how Ayre was capable of communicating with him during that process; So as much as I like her, she's quite literally a parasite since its not just being part of the machine that communicates with him, but she's directly part of whatever his body is (although that sequence also shows a lot of technology and stuff? so he's also got a good amount of cybernetics if nothing else).
A few things are hinted or implied, but they apply less to 621 specifically and more to the line of mercenaries supplied by the unnamed broker/doctor Walter talks to in the story trailer: that being that they are kept in cryo-stasis between contracts and trafficked like property.
We can further imply from bits of in-game discussion that 621 may theoretically be able to save up enough COAM to reverse their surgery and "go back to normal". This also tends to negate the "chili bag" and "brain in a jar" style theories about them, unless one assumes the doctor keeps the intact human body we see in the trailers on deep freeze for when they return.
Aside from this, everything beyond it is pure speculation. 621's identity is obscured intentionally as it is in every AC title so they can be whoever you want them to be beyond whatever is directly told to you (ex: the only detail in Master of Arena that is canon to the game is that the player's Raven is on a revenge quest after their family was killed, and in AC4 the canon is that you're a NEXT who works for Anatolia after being rescued from a nearly fatal incident).
