Those are the only two options that would make sense.
Some of them are just humans with large amounts of cybernetics, and even retain a completely human-looking appearance. The process even halts aging, and one augmented human was noted to still look in their 40s when they were actually 90+.
Other augmented humans though are said to be nothing more than brains in jars, especially when their body rejects the cybernetics.
So... depends, there's no one singular answer.
As for what they do to these AC pilots is anybody's guess. Mine is that they undergo some VERY excruciating mechanical and genetic procedures to make any of this possible. These pilots have either lived very long, physically grueling lives under the merciless direction of their handlers, or they're in peak condition, making them AC combat superstars in their own rights, worthy of their rank.
Most work probably goes into the brain, but I believe there's either physical augmentation or at least some operation done to make the vital organs more sturdy (or less vital).
When you're flung across the sea at great speed at the end of chapter 2, I don't think a normal human would've survived the G force.
A very, very rich Human+ can afford to keep a cybernetic human body.
Our character in AC6 is just an enslaved merc/soldier though. They don't have that luxury. Yet. We're told immediately that even a fried brain like us can buy their life back with enough money. It's just a matter of surviving long enough to earn that money.
My only question is: do they still go around like normal people or they are fixated on the PC.
All the plot and lore seems to indicate they just stay in the AC, reading messags and such.
Good luck living long enough as a pilot to get that rich though.
I'm pretty sure they do get outside and can act relatively like normal people. When 621 gets captured it's implied that his mech is taken away, forcing him to find an old rusty ac to escape
He's supposed to be super old (according to his Arena profile, he was around when the Fires of Ibis happened half a century ago!), but sounds like he's in his 30s-40s.
