That would be pretty impressive as you have to beat the game 3 times, with the 3rd time being on the hardest possible difficulty.
I guarantee there's people that used an achievement unlocker though (because there's always some with every steam game)

More than likely, I haven't played the game 24/7 since launch, but have completed all Main Story and DLC achievements, just on my 2nd playthrough to finish the last 2 achievements NG+ & Ultimate Difficulty at the same time

I thought it was just 2, first playthrough on whatever to unlock both NG+ and Ultimate difficulty, and then your second playthrough is basically just for those 2 achievements.

Lol Damn. I got the game a few days late but have been playing most of the time and am only in Act 3 on Spectacular, first playthrough. (every side mission, challenge, etc done so far though)

Everything I read gave me the impression you have to beat New Game+ to unlock Ultimate. If it IS in fact only 2, that'd be nice as I'm eager for Ultimate with how much Spectacular has me trying hard. (admittedly I'm stubborn and hate switching Gadgets, so I almost exclusively use Shooters and only recently started using Bombs more)

I'm still on my 1st play, I'm 91% on spectacular and did all the side stuff, I do have the secret photo locations and some other minor achievements but I think I'll leave the photos for NG+ because it will be a chore to do them all over again on NG+ .

I have. You actually only need to 100% the game once, then do the story missions only on NG+ and ultimate difficulty. The game does not need max rating on the Screwball challenges though, only spectacular for the achievements.

Edit: All legit as well, no mods, no nothing. Just vanilla from the start, then I took a few hours just to maul the story on NG+ ultimate for those last two achievements.

Exactly what I did, 100% my first playthrough which was the longest as collecting everything and especially finding all those secret photos only earning a suit and no achievement or bonus is a pity, then just skipped all cutscenes possible as well as turning off QTE in settings, doing NG+ Ultimate Difficulty, while pretty much only using Massive Suit Damage Power, and the 2 Suit Mods that buff damage helps speed along bosses also

I'm 77/78, can't figure out what that last one is. Thought it would be one of those get every achievement in the game achievements but I guess not.
